Purpose

To clearly define the importance of, and procedure for signing, KidHub’s permission form and waiver.

Policy

We require a signed permission form and waiver for each child that is registered at each of our programs.

Procedure

To sign the permission form and waiver, go to the Forms & Reports section in your Kindertales Account.

<aside> ❗ A signed permission form and waiver is required for attendance. We will not be able to accept your child into our care until we receive it.

Responsibilities

• Parents are responsible for reading, understanding, and signing our permission form and waiver for their children prior to attendance at a new program.
